After signing up for the course, you will be prompted to complete an intake form and initial baseline questionnaire in order to assess a baseline for your emotional, mental, and physical well-being at the start of this course. It will be presented again at course completion as a means to establish improved outcomes and future recommendations.
Data will be provided to your therapist at your request for use in your individual, couples, or relationship therapy sessions.
